Template for surveying players about encounters

This got me thinking about the sorts of options that a GM/DM might present to a gaming group in the form of a menu. It also allows for a wonderful and intuitive feedback mechanism that the DM/GM can use to solicit input from their group. These entries are, admittedly, rather generic and not balanced for any particular group.
